New cases

To open a new case, the following form must be completed. See details below the image.

The following information must be included in the form:

General

  1. Case Title: Provide a short description title for your case.
  2. Case Type: Select an option from the dropdown menu.
  3. Subject: Select an option from the dropdown menu.
  4. Description: Provide as much information as possible regarding the scenario you want to resolve.
  5. Attach a file: Attach screenshots, files, or any other documentation that may assist in resolving your request.

Partner Details

  1. Program Membership: Specify which program.
  2. Submitter: Your name.
  3. Dispute Status: Select an option from the dropdown menu.
  4. First Response SLA Status: Select an option from the dropdown menu.
  5. Is Escalated: Select an option from the dropdown menu.
  6. Milestone Date: Enter the date the milestone was achieved. 

Select Submit when the form has been completed.
